Please follow the below steps to contract with H & N Logistics, LLC.

1. Review Contract Manual

2.  Fill out and return the Broker/Carrier or Broker/Broker Contract

3.  Fill out and return Trailer Interchange Contract if you are interseted in pulling an H & N Trailer

4.  Fill out and return W-9 Form  *If you are an LLC you must enter your tax classification code*

5.  Fill out and return Automatic Direct Deposit Form

6. Fill out and return Carrier Profile Document

7.  Include a Certificate of Insurance naming H & N Logistics, LLC as additional insured

8. Include a copy of your Authority
